We strive to ensure a continuity of care and support services, bridging the gap for a smooth transition to adult life.

We are a specialist independent provider of transitional care, supported accommodation and services for Children and Young People aged 12+. Our unique and innovative range of support services provide an effective staged transition to adult life - making the bridge to independent living easier and achievable for the Children and Young People in our care.

Quality Standards

Progressive Care strives to ensure all of our care and support provided is underpinned by the nine quality standards as laid out by the Department for Education (2015).

‘Quality and purpose of care standard’

Progressive Care ensures all young people benefit from bespoke and tailored packages of care that are shaped around the young person and based upon their individual needs. We believe that children and young people should only live in quality environments, which are both homely and therapeutic in nature and representative of family homes.

'Children’s views, wishes and feeling standard’

We ensure children and young people are consulted in every possible area. We strive and endeavour to listen and learn from this consultation process and allow this to shape our current and future practice. We ensure that children and young people have access to a child centred residents guide and have access to an independent advocate to champion their rights and wishes further should they wish.

‘Education standard’

Within our homes we implement individualised support packages for each young person in order to promote their education and assist them to learn and achieve. We believe that each young person requires an individual approach to their education, this may involve a mainstream school, require a specialist education provision being identified, or conversely the home may have to promote opportunities for them to be educated on a more informal basis.

‘Enjoyment and achievement standard’

Young people are consistently encouraged to engage in activities, interests and hobbies in order to meet their physical, social, intellectual and cultural needs. At Progressive Care children and young people are furthermore encouraged to participate in youth clubs and local sports and community clubs to help them engage with peers from their local demographic area and assist in championing social inclusion.

‘Health and well-being standard’

Children and young people’s health and well-being is consistently promoted and furthermore prioritised within all our homes and communities. We ensure all our children and young people have access to local health amenities and are looked after and cared for by a staffing team that understand their individual health needs, and how they must be met. In all our homes and communities, healthy eating and a healthy lifestyle are championed and we ask and expect all our staff to act as appropriate role models to our children and young people; and reinforce these important values of healthy living.

'Positive relationships standard'

We encourage our children and young people to develop and sustain positive and meaningful relationships with our staff, fellow young people and within our local communities. To help facilitate these relationships young people benefit from individual and tailored behaviour management plans to help meet their emotional and intellectual needs. We believe an essential element to supporting children and young people and their presenting behaviours is establishing mutual trust and respect. This process is a fundamental priority and aim of all placements we provide at Progressive Care.

'Protection of children standard'

Our fundamental aim is to protect and ensure the safety of all children and young people in our care. To assist us in facilitating this we utilise a thorough and extensive risk management process and adopt a multidisciplinary approach to ensure that all agencies are actively communicating and working together in the best interests of safeguarding the child or young person. Progressive Care has extensive and effective preventative and proactive measures, as well as policies and procedures in place for safeguarding young people within our care, which all members of staff are firmly aware of.

'Leadership and management standard'

Progressive Care believe in employing a quality workforce that ensures children and young people are cared for by well trained, qualified and experienced members of staff. We continue to place an emphasis on empowering leaders within the organisation who each possess the ability to inspire and motivate those around them to help  to establish and promote a culture of learning and continued professional development within all our homes. We believe that inspiration should not stop with staffing, and that children and young people should be inspired by their individual key workers and other members of the staffing team, to reach and fulfil their potential and to set targets and goals to strive for in later life.

'Care planning standard'

At the very core of our organisation is promoting positive outcomes wherever possible for children and young people. To facilitate this process we ensure we have robust quality assurance processes in place on admission. This ensures we receive all the relevant paperwork and historical information necessary to complete accurate baseline assessments on children and young people, from which future outcomes can then be measured against. Progressive Care places an emphasis on completing accurate and robust documentation that is clear and consistent in all children and young people’s records. We ensure this is amended and updated accordingly in order to provide current assessments of circumstances and need, which may then impact on care delivery. 

Services

We offer a broad range of services, tailored to meet the indivduals needs. Our services consist of short-term intervention and ongoing assessment. Through a valued and varied dynamic setting we provide special medium to long-term placements, for vulnerable Children and Young People where positive outcomes are the aim.

Early intervention and a detailed assessment forms the person centred plan. This, together with appropriate individualised care, ensures preparation for independence, enabling people to achieve their full potential.

Pathways® Residential Children’s Homes

 

We have homes based in Manchester and Sheffield. We provide care and support to Children and Young People aged 12-17 years old, who may have emotional, social and/or behavioural difficulties, be suffering from crisis or trauma or have mild to moderate Learning Disabilities.

As an organisation we aim to minimise the potential amount of placements any Child or Young Person may endure. We have a wide range of services and often a child has progressed through our children’s home into our supported provisions before moving on to independent living.

The Children and Young People are an integral part in the development of the service and the day-to-day running of the home. We are a dynamic organisation and ensure that all the needs of any service user are paramount, and that their wishes are taken into consideration.

Continuity of care...

This service is for Young People who are preparing for their move to supported living. We work closely with our partner agencies to ensure that each young person has a workable person centred Pathway® plan.

Bridging the Gap between Residential Care and Supported Living

We listen to the young people and tailor the support that we provide to meet their individual needs. We believe that each young person has the right to set their own timetable to enable them to achieve a successful transition to Supported independent living.

Parents would not want their children to be left on their own with limited levels of support. In order to give their child the best head start in life, any Parent will continue to provide appropriate levels of support, to ensure they succeed on their pathway to independence. We simply replicate this process as closely as we can to ensure a positive transition to independence is achieved.

Our services are designed to support and develop young people’s skills to ensure their social inclusion and productive and active engagement within their communities.

We at Progressive Care understand this stage in a Young Person’s life can be daunting. Our service is flexible but yet targeted in meeting the specific needs of young people at this vulnerable time. We enable a positive transition to Supported Living through:

Continuous Assessment

Continuous assessment ensures appropriate intervention and support. The information gathered through a detailed assessment will be built into the individuals Pathway plan and consistently shared with the young person, their social worker and other agencies.

The Pathway® Plan

The Pathway® plan will be an ongoing working document which has input from the Young Person, their social worker, their key worker and any other professional involved. Throughout the placement, monthly planning meetings will ensure that the Outcome Plan and the young person’s progress towards independence are monitored

Practical Skills

With a person-centred plan we aim to bring stability to a young person’s life. We realise there are barriers to achieving and we aim to overcome these barriers.

The development of practical skills is focussed on not only removing those barriers to achieving successful outcomes but also relate to practical situations in everyday life in order to develop the requisite maturity necessary for positive transition to Supported Living.

The ability for a young person to understand the skills and issues relating to housing, education, money and the support systems creates a meaningful platform for adult life.

Pathways® Sheffield

 

Pathways® 204

Pathways® Children and Young People

Pathways® 204 is a medium/long-term home for Children and Young People aged 12 – 17 yrs with emotional and behavioural difficulties. It is a registered 4 bedded facility and as a responsible provider of services; care and consideration is taken to involve everyone when considering potential placements. We ensure that we are sensitive, dynamic and caring towards Children and Young People's thoughts and feelings and this begins with getting as much information as we can during the assessment process.

We ensure that all Children and Young People are empowered to participate in their care plan and take ownership of any potential outcomes.  Staff at Pathways® 204 work in conjunction with education staff in order to gain access for our Children and Young People into learning, creativity and lifelong opportunities. 

At Pathways® 204 preparation for independent living is given high priority, not only in ensuring that Children and Young People have the practical tasks to enable them to move on, but also that they know where to go for help when they need it.  Children and Young People are encouraged to build appropriate social networks outside of the home, as friends and leisure time are particularly important to teenagers.

Pathways® 206

This is a 4 bed provision. There have been many examples of Pathways® being able to take on and sustain some very challenging Children and Young People.  The home has a proven track record of accurately assessing Children and Young People’s needs and implementing individual programmes of care.

The staffing at the home is reflective of young peoples needs. For Young People that require further support we have the capacity to provide 1:1 staffing levels, to ensure their needs are met.

Our staff have knowledge, skills and expertise in working with Young People, ensuring that their wishes and feelings are taken seriously when planning their future. This consultation process results in Young People developing sound professional relationships with staff, enabling them to make transitional steps into future independent living.

Pathways® Stalybridge offers accommodation, including a studio flat, for up to 6 Children and Young People, who are moving towards independent living. We specialise in helping children with complex emotional and behavioral needs in maximising their potential. The home is staffed 24 hours a day and there is a Manager on call at all times to ensure Children and Young People have all the support they need.

Supported Living 16+

We are a specialist independent provider of transitional support services and accommodation for Young People aged 16+ to Adults.

Our unique and innovative range of complimentary support services provide an effective 3 staged transition to adult life, making the bridge to independent living more achievable for the young people we support. Transition is facilitated through continuous assessment and individualised packages of support.

The 3 Stages consist of:

Stage 1 - Enhanced Supported Living aged 16 - 18
Stage 2 - Core Semi Supported Living aged 16 - 18                                     
Stage 3 - Independent Living aged 18+ with Floating Support

Stage 1 - Enhanced Supported Living (Aged 16-18)

A specialist, tailored service for Young People aged 16+ moving on from residential or foster care. We at Progressive Care understand that this stage in a young person’s life can be daunting. This service is delivered from either Handsworth (Sheffield), Crimicar Lane (Sheffield) or Carlton Mews (Barnsley).

Our services are flexible but yet targeted in meeting the specific needs of Young People at this vulnerable time. We enable a positive transition to independent living through:

Stage 2 - Core Semi-Supported Living (Aged 16-18)

This Core group living service provides the next transitional step to independent living from the Enhanced service, offering continuity and flexibility for Young People. Our services are available at Woodside (Sheffield).

With the peace of mind of having fall-back support from the Enhanced service, support is tailored to meet the individual needs as a direct result of goals being achieved.

Stage 3 - Independent Living with Floating Support

Reasonable parents would not want their children to be left on their own and unsupported when they leave their family home, they would provide whatever level of support is needed to ensure that their children are given every possible chance to succeed. 

CQC Registered Floating Support Service

Having been supported through the previous 2 stages, our stage 3 is delivered through our CQC registered Floating Support Service. 

Our staff are flexible and adaptable providing continuity and consistency to Young People as they move through the 3 stages of support.

Through a quality assessment framework we provide packages of support hours to enable Young People to be assisted in the development of their own life plan and where they will be encouraged to make independent choices, learn from experience and to be a confident and active member of the community.

Woodside (16+ provision)

Introduction

Woodside offers Supported Accommodation for up to 9 Young People in either a studio or one-bedroom apartments. Woodside is staffed 24/7, including waking night staff and access to on-call support.

Our staff have the skills, knowledge and expertise in working with Young People and have developed excellent relationships with professionals who have an input with Young People. 

Our Aim

Our aim is to help Young People to gain the skills they need to lead a full and happy life as independent adults. The service is tailored to meet the needs of each individual, and listening to the views and wishes of Young People is an essential part of our work, to support the achievement of outcomes.

Young People are encouraged to take part in local community activities and strong links have been established with the local community. We encourage and support Young People to move into Further Education, Training and Employment.
